Cases

1.A 52 year old man presented with shortness of breath and coughing up small volumes of blood. Clinical examination revealed multiple dilated veins around the neck. A chest x-ray demonstrated a tumor mass involving the upper part of right lung and elevated right dome of diaphragm. Based on your anatomical knowledge answer the following questions
  • Compression of which structure in the neck by the tumor is responsible for dilated veins around the neck
  • Mention the extent and the relation in the, mediastinum
  • Briefly describe the other structures related to the mediastinal surface of the right lung
  • Injury to which structure is responsible for elevated diaphragm on the right side
  • Briefly describe the bronco-pulmonary segments of the lung and its applied importance(1+2+3+1+3=10)

2. A 70 year old man on walking uphill feels sudden onset of severe chest pain radiating to the medial side of left arm associated with tiredness and sweating. He gives a history of similar attacks and was on treatment. With your knowledge in Anatomy answer the following questions.
  • Name the organ affected
  • Give a brief account of its arterial supply.
  • Mention the reason for the radiation of pain.
  • Name the covering of the organ and give their nerve supply. (1+4+2+3=10 marks)   

 
3.A 48year old woman came to the surgery out-patient department with complaint of a lump in her right breast. On examination, surgeon found that the ump was hard and painless and in the upper outer quadrant of right breast. Axillary lymph nodes on the right side where enlarged and right breast was not mobile.  Histopathological examination revealed malignancy of right breast. With your knowledge in anatomy answer the following questions:
  • What is the normal extend of the breast both vertically and horizontally
  • Name three muscles related to the base of the breast
  • What could be the reason for loss of mobility of breast in this condition
  • Describe the lymphatic drainage of the breast
  • Describe the arterial supply of the breast(1+1.5+1+3.5+3=10)
